UroSculpt™ Certification Levels and Practice Membership
UroSculpt™ certification is offered at three levels. Certified is earned through the online course or the challenge exam, and Advanced Certified through in-person training; Master Certified is awarded by invitation. Each credential is valid for 2 years and is renewed by examination. Practice Membership is a separate, optional monthly plan for providers who would like patient leads and marketing support.
The three certification levels
Certified
Certified status is earned by completing the online course ($1,995) and passing the exam with a score of 80%, or by passing the challenge exam ($1,995) with a score of 85%. It entitles you to market yourself as a UroSculpt™ Certified provider and includes a listing in the directory. The certificate is issued once your license has been verified.
Advanced Certified
Advanced Certified status is earned by completing in-person training ($6,990), which includes UroFirm™ penile botox training and two years of Practice Membership. Advanced Certified providers sign the provider affiliation agreement. If you already hold the Certified level, your online course tuition is credited toward the training day.
Master Certified
Master Certified status is awarded by invitation to Advanced Certified providers with a demonstrated case volume and outcomes who join the Upsize™ network. Master Certified providers carry a Master badge in the directory, may receive referrals from the Upsize™ network and may serve as training faculty. The designation is a recognition and is separate from Practice Membership. Please contact us if you would like to be considered.
Renewing your UroSculpt™ credential
Every credential is valid for 2 years from the date of issue. Renewal requires a fee of $995 and a passing result on the recertification exam in the provider portal, which covers any changes to the curriculum since you were certified. The portal sends an email reminder before your credential expires. Renewal preserves your certification level and directory listing, and it is never tied to a monthly subscription.
What certification includes, and what Practice Membership adds
Certification includes the core benefits below with no monthly fee. Current UroSculpt™ certification includes the right to describe yourself as UroSculpt™ Certified and to use the mark in your own advertising, with no separate fee for the mark. Certification must be renewed by recertification exam every 2 years to remain current and listed; if it lapses, the right to use the mark and the directory listing lapse with it. Practice Membership costs $149 a month, can be canceled at any time without affecting your credential, and is included for the first two years with in-person training.

Patient leads: while the number of providers is growing, leads are sent to eligible members at no additional charge. Once a market has enough providers, leads in that market will cost $100 each, and members will receive at least 30 days' written notice before that change takes effect. You can start or cancel your membership directly in the provider portal.
How to become certified
Begin at the level that suits your experience. You can enroll in the online course immediately, book in-person training once we have agreed on a date with you, or apply for the challenge exam if you are already an experienced injector.
