P-Shot vs Penile Filler: Which Should You Learn First?

Providers entering men's sexual health usually meet two injectable modalities first: platelet-rich plasma (the trademarked P-Shot®) and hyaluronic acid dermal filler for girth enhancement. They treat different problems, follow different evidence, sit in different training ecosystems, and produce very different practice economics. This guide compares them side by side so you can decide which training to pursue — or whether to offer both.

The Short Answer

They are complementary, not competing. PRP (P-Shot®) is positioned as a functional/regenerative treatment — most commonly offered for erectile function support — with modest per-treatment revenue and an evidence base that is still maturing. Penis filler (HA girth enhancement) is an aesthetic procedure with an immediately visible, measurable result, substantially higher per-patient revenue, and a more direct patient value proposition. Most providers who offer both report that girth enhancement anchors the revenue while functional treatments round out the service menu. If you are choosing one training to start with, the economics favor filler; if you are building a full men's-health menu, plan for both.

The Two Modalities

What Each Procedure Actually Is

P-Shot® (PRP Injection)

Platelet-rich plasma is drawn from the patient's own blood, concentrated by centrifuge, and injected into the corpora cavernosa and glans. It is marketed primarily for erectile function support, and sometimes for Peyronie's disease or sensitivity. The published evidence for PRP in erectile dysfunction is mixed and still developing — some small trials show benefit as an adjunct, while size-related claims have little supporting data. The P-Shot® name itself is trademarked: to market under that name, providers train through and license the protocol from the trademark holder's organization, with ongoing license fees.

Penis Filler (HA Girth Enhancement)

Hyaluronic acid dermal filler is placed between the skin and dartos fascia in a controlled, layered pattern to increase penile girth. The result is immediate, visible, and measurable — typically a 0.25–2 inch girth increase depending on volume — and lasts roughly 18–24 months before maintenance. It is an aesthetic procedure with a clear before-and-after, which makes the patient conversation straightforward. Training is certification-based rather than name-licensed: UroSculpt™ certification includes the proprietary technique, complication management (including hyaluronidase reversal), and provider-network benefits, with no ongoing licensing fee required to perform the procedure.

Training Paths

Two Very Different Training Ecosystems

The P-Shot® world is a licensed-trademark ecosystem: you train with the organization that owns the name, and you pay ongoing license fees to market the procedure under that brand. The training itself is typically short — the injection is technically simple — and the license is really what you are buying, since PRP injections themselves are not proprietary.

Penis filler training is a certification model. The skill itself is the product: filler placement in the penis is genuinely technique-sensitive, and outcomes track directly with how the injector was trained. With conventional freehand technique, 50% or more of patients need a follow-up touch-up correction for lumps or migration; the UroSculpt™ technique cuts that to roughly 1 in 7. That is why the certification carries weight with malpractice carriers, and why the training investment is larger than a PRP course — you are learning a harder procedure with a much larger revenue ceiling.

There is no conflict between the two paths. A number of UroSculpt™ certified providers also offer PRP treatments. The practical sequencing question is which one moves your practice economics first — and per procedure, girth enhancement produces roughly two to four times the revenue of a PRP treatment at comparable chair time. Common Questions

P-Shot vs Penis Filler — Provider FAQ

?
Can I offer both the P-Shot and penis filler in the same practice?

Yes, and many providers do. They serve different patient motivations — function versus size — and they are often cross-sold: a girth-enhancement patient is frequently interested in functional treatments and vice versa. The trainings are independent; completing one does not restrict the other.

?
Does P-Shot training qualify me to perform penis filler?

No. PRP injection and dermal-filler girth enhancement are different procedures with different anatomy targets, different complication profiles, and different technique demands. Filler placement is substantially more technique-sensitive — vascular complication avoidance, plane selection, and volume distribution all require dedicated training. Malpractice carriers generally treat them as separate privileges as well.

?
Which produces more revenue for a practice?

Per patient, girth enhancement: network recommended pricing runs $3,990–$8,900 per treatment plan versus roughly $1,500–$2,200 for a typical PRP treatment, at comparable chair time. PRP's advantage is repeat treatments; filler's is revenue per visit and a visible result that markets itself. The revenue analysis covers realistic ramp and volume modeling.

?
Is there a combined training for both?

UroSculpt™ certification covers HA girth enhancement, and in-person cohorts include UroFirm™ penile neuromodulator (botox) training at no additional charge — a functional modality with a published randomized-controlled-trial protocol. PRP training is available separately through the P-Shot® licensing organization if you want to add it under that trade name.
