Penis Botox for erectile dysfunction

UroFirm™ is an in-office treatment for erectile dysfunction: a single injection intended to provide firmer, more reliable erections without the need to time a pill. The treatment takes 20 to 30 minutes, and the effect of one treatment lasts up to six months.

How it works

An erection depends on relaxation of the smooth muscle within the erectile tissue, which allows blood to flow in. Botulinum toxin blocks a signal that causes this muscle to contract, so that more blood can reach the tissue. Oral ED medications and Trimix injections act by a different mechanism, dilating the blood vessels, and must be taken before each sexual encounter.

The treatment consists of 100 units of Botox, or an equivalent dose of another botulinum toxin such as Letybo, Dysport or Xeomin, which your provider injects directly into the erectile tissue (the corpora cavernosa).

Clinical evidence

In a randomized, placebo-controlled trial of men whose erectile dysfunction had not responded adequately to oral medication alone, the addition of a single injection improved erections in 65 percent of patients, and 53 percent achieved an erection firm enough for penetration, compared with one man in the placebo group. No complications were reported.[1]

A second randomized trial, of 176 men, found that the effect of a single 100-unit treatment lasted up to six months.[2]

What to expect

The appointment takes 20 to 30 minutes. Your provider reviews your medical history and any treatment you currently use for erectile dysfunction, you sign a consent form, and the medication is then given as a single injection. Most men resume their usual activities immediately afterward.

The effect is not immediate, because the medication takes time to act on the smooth muscle. Improvement may begin within two weeks and continues to develop over the following weeks. One treatment lasts up to six months, during which there are no pills to time, and it can be repeated when the effect wears off.

Who is a candidate

UroFirm™ is intended for men with erectile dysfunction, including those for whom oral medication alone has not been sufficiently effective. Candidates should be adults in good general health, with no infection in the treatment area and realistic expectations: a single injection can provide firmer, more reliable erections for up to six months, but it is not a cure.

Side effects

Common side effects are mild tenderness and minor bruising at the injection site, which usually resolve within a few days. Rare complications include infection and an erection lasting more than four hours (priapism), which is a medical emergency. If an erection lasts more than four hours, seek emergency medical care immediately.

Botulinum toxin is FDA-approved for other indications, and its use for erectile dysfunction is off-label. Results vary from patient to patient.

Price

Treatment costs about $1,890, although each provider sets their own price. One treatment lasts up to six months. Most clinics offer monthly payment plans through Cherry or CareCredit. How much does it cost?

How long does it last?

A single treatment lasts up to six months. Improvement may begin within two weeks, and the treatment can be repeated when the effect wears off.

Is it safe?

Common side effects are mild tenderness and minor bruising at the injection site, which usually resolve within a few days. Rare complications include infection and an erection lasting more than four hours (priapism), which is a medical emergency. The use of botulinum toxin for erectile dysfunction is off-label.

Is it the same as cosmetic Botox?

It belongs to the same class of medication, but it is injected into the erectile tissue rather than into the facial muscles, using a technique in which certified providers have been trained.

Can it be combined with penis filler?

Yes. Filler increases girth, while UroFirm™ addresses erection quality, and some men choose to have both as part of one treatment plan. Your provider can discuss this with you at the consultation.
