How the penis filler procedure works

Penis filler is hyaluronic acid injected into the subdermal layer, just beneath the skin of the shaft, to increase girth. The procedure is performed in a single office visit under a local anesthetic.

The procedure, step by step

The following describes a typical treatment. Your provider's own plan, aftercare instructions and follow-up schedule may differ, and theirs are the ones to follow.

  1. Consultation

Treatment begins with a consultation. Your provider reviews your medical history, examines you, talks you through your goals and shows you before and after photos, then determines the appropriate amount of filler for you. This is where you ask questions and discuss results, risks and price. Full list of risks and complications →

  2. Preparation

Before treatment, you may be asked to stop any blood-thinning medications for several days, as they increase the likelihood of bruising. On the day of the procedure you will sign the consent forms, and photographs are taken for your medical record so that the result can be compared with your baseline appearance.

  3. Local anesthesia

A local anesthetic is used to numb the treatment area; you remain awake but typically feel little more than pressure. Most hyaluronic acid fillers also contain lidocaine, which provides additional anesthesia as the filler is injected.

  4. The injection

Working through a fine cannula, your provider injects the filler at several points along the shaft and distributes it evenly in the subdermal layer, so that the result is smooth. The injection takes 30 to 60 minutes, depending on the volume used.

  5. After the procedure

Your provider assesses the result and reviews the aftercare instructions with you, which are also provided in writing. Most men leave the office 15 to 30 minutes after completion of the procedure, and the added girth is visible immediately.

  6. Follow-up

Your provider will advise you whether a follow-up visit is needed. Where one is scheduled, it serves to confirm that healing is progressing normally or to assess the final result once the swelling has resolved.

Recovery

During the first few days, swelling, bruising and some tenderness are to be expected, and an ice pack can help relieve them. The swelling typically peaks at about two to three days and then gradually diminishes; in the meantime, the penis appears larger than it will once the swelling has resolved. Most men do not need to take time off work and are able to resume normal activity within a few days.

The principal restriction in the first week concerns sexual activity, which should be avoided for at least one week so that the filler can settle where it was placed. Beyond this, most providers do not use compression sleeves, wraps or molding routines. By the second week most of the swelling and bruising has resolved, and the final result is typically evident at about four weeks. Hot tubs and steam rooms should be avoided for four weeks.

Expected results

Penis filler increases girth by up to 0.5 inch with 5 ml, up to 1 inch with 10 ml and up to 1.5 inches with 15 ml. The larger volumes also add a modest amount of flaccid length; erect length does not change. The result lasts about two years, and the filler can be dissolved with hyaluronidase if you wish to reverse it sooner.[1][3]

These are typical results. Your own result will depend on your starting size and the volume of filler used, and your provider will explain what you can expect at your consultation.

Who is a candidate

Suitable candidates are adults in good general health who want greater girth and have realistic expectations of what filler can and cannot achieve: it does not increase erect length, and the result is not permanent. Penile anatomy should be normal, with no significant curvature or deformity that requires treatment first, and there should be no infection or skin condition in the treatment area. Patients must also be able to follow the aftercare instructions, which chiefly involve abstaining from sexual activity for one week.

Side effects and risks

Common and temporary

Most patients have some swelling and bruising in the treatment area, which usually resolves within one to two weeks. The area may be tender during healing. Slight unevenness while the filler settles is common, as is mild redness, and these effects generally resolve without intervention. Rarely, numbness or altered sensation can occur during healing, but it is temporary.[1]

Uncommon: infection, lumps, migration of the filler and inflammatory reactions.

Rare: blockage of a blood vessel at the injection site (vascular occlusion).

Extremely rare: filler reaching the lungs (pulmonary embolism).

Hyaluronic acid fillers are FDA-approved for use in the face; their use in the penis is off-label. Results vary from patient to patient.[2]
