Why Scrotum Appearance Changes With Age (And What You Can Do)

· 4 min read

Nobody warns you that your scrotum ages. But if you’ve noticed changes over the years (more wrinkles, different hang, changed texture) you’re not imagining things. Understanding what’s normal, what causes these changes, and what options exist helps you decide whether to do anything about it.

What Changes and Why

Skin Changes

Like skin elsewhere on your body, scrotal skin loses elasticity over time. Collagen and elastin—the proteins that keep skin firm and resilient—decrease with age. The result is looser, more wrinkled skin that doesn’t “snap back” like it used to.

Sun exposure accelerates these changes in skin that’s been exposed. While the scrotum typically gets less sun than other areas, overall skin aging affects the whole body.

Muscle Tone Changes

The dartos muscle that controls scrotal contraction loses some tone with age. This can actually go two ways: some men find their scrotum hangs lower because the muscle doesn’t contract as firmly. Others find it becomes more chronically contracted as the tissue tightens.

Changes in “Hang”

Many men notice their scrotum hangs differently than it did when they were younger. This can mean:

  • Hanging lower due to reduced muscle tone and skin elasticity
  • Or appearing more retracted if tissue has tightened
  • More variability—sometimes hanging, sometimes tight—depending on conditions

Texture and Appearance

The smooth appearance of youth gives way to more textured, wrinkled skin. Color may change slightly. Visible veins may become more prominent. Hair patterns often change as well—graying, thinning, or becoming coarser.

What’s Normal vs. What Needs Attention

Most changes are purely cosmetic and normal aging. However, some changes warrant medical evaluation:

See a doctor if you notice:

  • Lumps or masses in the scrotum or testicles
  • Significant asymmetry that’s new (one side much larger)
  • Pain or tenderness that persists
  • Skin changes that could indicate dermatological conditions (rashes, sores, discoloration)
  • Swelling that came on suddenly

These could indicate conditions requiring treatment unrelated to normal aging.

Normal aging includes:

  • Gradual changes in skin texture and elasticity
  • Changes in hang that developed slowly over years
  • More wrinkling
  • Mild changes in appearance that aren’t associated with symptoms

Options If You Want to Address Changes

Scrotox

Botulinum toxin injected into the scrotum relaxes the dartos muscle, creating a smoother appearance and allowing the scrotum to hang lower. This is the most popular treatment for aesthetic scrotal concerns.

What it addresses: Wrinkled, contracted appearance. Creates smoother, fuller look.

What it doesn’t address: Loose, sagging skin from lost elasticity. Won’t “tighten” skin that’s become too lax.

Scrotal Tightening (Scrotoplasty)

Surgical removal of excess scrotal skin for men whose scrotum has become too lax or hangs lower than they’d like. This is more invasive than Scrotox but addresses a different problem.

What it addresses: Excess skin, excessive sagging.

What it doesn’t address: Wrinkled, contracted appearance (Scrotox better for this).

This is a surgical procedure with longer recovery than injectable treatments.

General Skin Health

While you can’t reverse aging, maintaining skin health helps slow further deterioration:

  • Adequate hydration
  • Sun protection for areas that are exposed
  • Moisturizing (the scrotum benefits from this too)
  • Not smoking (accelerates skin aging throughout the body)

The Confidence Factor

For many men, the issue isn’t really about the scrotum specifically—it’s about feeling like their body is showing age in ways they don’t like. The scrotum is just one visible sign among many.

Some questions worth considering:

  • Is this something that genuinely bothers you, or just something you’ve noticed?
  • Would addressing it actually make you feel better, or would you just find something else to focus on?
  • Are you comfortable with the idea of ongoing maintenance (for Scrotox) or surgery (for scrotoplasty)?

There’s no wrong answer. Some men feel genuine distress about these changes and benefit from treatment. Others notice changes but don’t feel they warrant intervention. Both responses are valid.

Talking to Partners

If you’re self-conscious about aging changes, it’s worth noting that partners are often far less focused on these details than you are. What you notice in close inspection may be invisible to someone who sees you in the context of the whole person and the relationship.

That said, if something bothers you, your feelings are valid regardless of whether a partner notices or cares. Aesthetic treatments are about how you feel about yourself, not just how others perceive you.

Making Decisions

If scrotal aging changes genuinely bother you, options exist. Start with understanding what’s actually changeable:

  • Wrinkled, contracted appearance → Scrotox can help
  • Excessive laxity and sagging → Surgical options exist
  • General appearance of aging skin → Limited options, but general skin health helps

Consultation with a provider experienced in men’s aesthetics can clarify what would actually address your specific concerns and what’s realistic to expect.

Aging is inevitable; how you feel about it and what you choose to do about it is personal. The goal is making informed decisions that align with what matters to you.
